Job Summary:

The Chronic Care Coordinator is responsible for functioning in an expanded role to facilitate care through interdisciplinary collaboration and care coordination.  Responsible for communicating with patients and caregivers to ensure improved care transitions, enhancing patient to provider communication to identify opportunities for care continuity, encouraging the patient’s self-management skills and understanding the healthcare system to guide and empower patients to participate in their own care plan.

Primary Job Responsibilities:

  1. Help grow the Chronic Care Management/Patient-Centered Medical Home Programs.
  2. Continually assess and provide suggestions to improve Care Plans.
  3. Document all patient care interactions in Electronic Health Record.
  4. Must have superb time management skills.
  5. Continually assess and improve relationships with all healthcare resources.
  6. Must have superb assessment skills and be able to relay those to the healthcare provider in a professional manner.
  7. Be able to float from the hospital and the clinic to help with the coordination of care upon discharge.
  8. Monitor our overdue testing and referrals and be contacting the patient to get them to comply with their orders.
  9. Form relationships with the local pharmacists for medication education.
  10. Perform call backs after provider appointments.
  11. Encouraging the providers to use available resources to help in plan of care.
  12. Must maintain confidentiality of patient’s protected health information.
  13. Must be adaptable to change and be able to work under pressure to meet deadlines.
  14. Must be able to read, understand, and act on government regulations for billable services.
  15. Work closely with people from all healthcare teams to provide a solid team for the patient and our Care Coordination Program.
  16. Continually assess, implement, and evaluate patient self-goals and goals set by the provider.
  17. Monitor patient compliance and form a plan of how to create compliance with patients.
  18. Be familiar with payer requirements.
  19. Be the patient’s biggest advocate.
  20. Identifies opportunities for health promotion and illness prevention.

Position is open to an Illinois Licensed LPN or RN

Hours: Full-Time, 8am-4:30pm – Locust Street Clinic
