New Patient Info

Thank you for choosing Memorial Hospital! We’re confident that you’ll find your experience here as comfortable and convenient as possible. Please help us help you by reviewing the new patient information below.

  1. Upon arrival, you may have someone drop you off at the front door or you may choose to park in the lot directly facing the main entrance. You will be warmly greeted by a volunteer at the Welcome Desk!
  2. To expedite the registration process, please call 217-357-8695 at least one day before your arrival. When calling please have the following information ready: Health Insurance cards with policy numbers; Employer’s name with whom you are insured and insured’s date of birth; Medical Provider’s name; Medical Provider’s written order form with the tests or services ordered and related diagnosis. When you call to pre-register, your Information will be reviewed and confirmed. Pre-registration allows you to go directly to the clinical area and bypass Patient Access.
  3. If there is a wait, we will work hard to make sure it is as short as possible. We invite you to relax and watch television. Or you may browse through educational material or a magazine. Memorial Hospital also offers wireless Internet access if you have brought a wireless laptop computer.
  4. If you are accompanied by a friend or family member, they may want to consider stopping by the courtyard cafeteria or Memorial Hospital Foundation gift shop. Snacks and beverages are always available in the cafeteria, where you will also find the serving hours for meal times. These areas might be particularly inviting if a friend is waiting for you during an outpatient procedure.
  5. At Memorial Hospital and Memorial Medical Clinics, we respect and support your comfort and privacy during medical exams.  Medical chaperones are available to be present when you are being examined by your provider.  If you would like a trained chaperone present during your exam, please ask your provider. Your well-being is our priority, we are here to ensure you feel at ease during your visit.
