Job Summary:

The Bilingual Patient Navigator position is responsible to assist in screening patient’s medical health needs and may on occasion assist with screening patient’s mental health needs by keeping other team members such as, but not limited to, the Physicians, Advanced Practice Nurses, Nurses, Behavioral Health Providers, Social Workers, and Support Staff apprised of both medical and behavioral health services available and/or offered to patients. The Bilingual Patient Navigator gathers information from patients or other team members and assists with connecting patients with the appropriate medical providers or behavioral health providers, if necessary. This position provides linkages to community resources or others that would be beneficial to the patients overall health.

Primary Job Responsibilities:

Documentation and Data Collection:

  • Assists with the documentation process of the assessment thoroughly and completely and in adherence with best practice standards as determined by the supervisor.
  • Provides accurate and timely documentation of patient/client information as directed by the supervisor for the patient/client record.
  • Assists with the maintenance of data bases and related records and provide information to the appropriate hospital/clinic personnel as well as outside agency contacts when directed by the supervisor.
  • Assists with data collection and scores for quality management reporting as directed by supervisor.

Interpreter:

  • Translate English language verbal and written communication to Spanish for Spanish-speaking patients/clients and their families.
  • Translate English language documents to Spanish when time permits, and situations are appropriate as directed by the supervisor.
  • Assist or serve as a liaison between non-English speaking communities and the organization.
  • Respond to concerns regarding language interpretation and communication for patients/residents and their families.

Education:

  • Involvement in assessment and making ongoing self-evaluation of interpreter skills.
  • As directed by supervisor will coordinate, develop, and distribute translated patient education materials, medical forms, etc.
  • Provide culture education to other employees.

Supportive Services:

  • Assist to identify patient and/or family needs and may make appropriate referrals to ensure integrated health care services and/or community resources are offered and utilized.
  • Assist with patient/client support and care follow-up as directed.

Community Patient/Client Coordination & Communication:

  • This position may assist the Marketing Department to provide outreach services messaging to the community, clients, patients and/or families by creating positive messages to attempt to engage the clients/patients in the appropriate services.
  • Collaborate with the integrated healthcare team or other service providers in trying to make or maintain contacts with hard-to-reach patients, clients, or families.
  • May assist or serve as a liaison with the Hispanic/Latino communities.
  • Apply a minimum of one health literacy method when communicating with patients/clients.

Job Specifications:

Education Requirements:

  1. Bachelor’s degree in Social Work, Social Services, Psychology, Sociology or related field of study or equivalent work experience preferred. High school diploma or GED required.
  2. Prefers experience in behavioral / mental health assessment and case management experience.
  3. Bilingual in English and Spanish required.

Hours: Monday-Friday, 8am-4:30pm – Location: Locust Street Clinic
