Diabetes Self-Management Education

Diabetes Self-Management Education is a comprehensive, individualized program to meet the needs of the person living with diabetes. The individual’s information is confidential and discussed only in a private appointment.

The education process is directed by your physician’s referral, in collaboration with our team of expert educators. Our team of educators includes a Registered Nurse and a Registered Dietitian who is a Certified Diabetes Educator, and a Registered Pharmacist.
