Memorial Hospital is proud to announce it has been recognized by the Illinois Rural Community Care Organization (IRCCO) with the Best in ACO (Accountable Care Organization) Medicare Wellness Visits 2025 Award. This award honors Memorial Hospital’s commitment to preventive healthcare and its success in encouraging Medicare beneficiaries to complete their Medicare Wellness Visits.

Medicare Wellness Visits play an important role in helping patients stay healthy by identifying potential health risks, closing gaps in care, and creating personalized plans that support long-term wellness. Through meaningful patient outreach, strong provider engagement, and a continued focus on preventive care, Memorial Hospital has helped patients take a proactive approach to their health while supporting the goals of the Medicare Shared Savings Program (MSSP).
