Primary Job Responsibilities:
- Lead the day-to-day functions of the unlicensed nursing staff, including directing and correcting work.
- Supervise resident care specialist on shift in documentation and work assignments.
- Have direct responsibility for coordination and delivery of residents nursing needs.
- Provide hands-on care to residents as directed.
- Receives Physician Orders and communicates with pharmacies to ensure prescribed medications are properly set up and available to the resident.
- Act as liaison between the physician, family, and other departments in the community.
- Foster self-respect and a feeling of worth in each resident by consistent kindness, understanding, and patience in their care.
- Attend and participate in continuing education programs designed to keep you abreast of changes in the profession.
- Consistently work cooperatively with residents, staff, volunteers, and families to ensure residents are receiving the best quality care.
- Assist in orientation for new employees to their duties according to the facility guidelines.
- Organize and maintain all records necessary, following facility policies and procedures.
- Participate in the Nursing on-call rotation [Full-Time: Mandatory / Part-time: On-Request]
- Follow all written and oral directions given by supervisor.
- Participate in all in-services and activities as required.
- Use proper nursing skills and techniques to ensure safety of self, residents, volunteers, visitors, and staff.
- Observe all facility policies and procedures, including but not limited to safety, infection control, residents’ rights, and those contained in the employee handbook.
- Maintain the confidentiality of all protected health information whether electronic, written, or oral exposed to either during the course of assigned duties or as a result of an incidental disclosure.
- Provides episodic and intermittent health promotion or disease prevention counseling and teaching self-care in meeting routine and special health care needs that can be done by other staff under the supervision of the registered nurse.
- Performs required documentation.
- Completes with DON quarterly evaluation of resident.
- Completes requested nursing assessments as directed.
Required Qualifications:
- Pass background check before and during employment
- Graduate from an approved school of nursing.
- Active CPR certification
- Currently licensed in Illinois as an LPN or RN.
- Able to follow written and oral directions.
- Demonstrate dependability, cooperation, and interest in the care of elderly.
- Display leadership qualities, good communication skills, and a desire to continuously learn.
- Possess the ability to communicate effectively and deal tactfully with personnel, residents, families, visitors, government agencies, and the general public.
Physical and Environmental Demands:
- Frequently: Sit, stand, and walk
- Frequently: Stoop, push and pull
- Occasionally: Squat, balance, kneel, crouch, and reach above shoulders
- Occasionally: Lift up to 50 lbs.
- Frequently: Lift up to 25 lbs.
- Regularly: Use hands for simple and firm grasping and fine manipulation.
- Occasionally: Exposure to hazardous chemicals such as disinfectants, cleaners, soap, etc.
- Occasionally: Exposure to latex gloves and other personal protective equipment.
- Occasionally: Exposure to blood/body fluids as required to provide resident care.

Wage Range:
LPN: $19.00 – $21.50/hour (Actual wage based on relevant experience)
RN: $21.00 – $23.50/hour (Actual wage based on relevant experience)
