Diabetes Self-Management Education
Diabetes Self-Management Education is a comprehensive, individualized program to meet the needs of the person living with diabetes. The individual’s information is confidential and discussed only in a private appointment.
The education process is directed by your physician’s referral, in collaboration with our team of expert educators. Our team of educators includes a Registered Nurse, who is a Certified Diabetes Educator and Technology Clinician, and a Registered Dietitian who is a Certified Diabetes Educator, a Registered Pharmacist and a Licensed Professional Counselor.
Certified through the American Diabetes Association since 2004, the program meets the standards of the ADA for a quality diabetes self-management education program, recertifying every four years and continuing to meet criteria for insurance coverage of services provided.
