Memorial Medical Clinics will be offering Sports Physicals again this year.

SCORE BIG WITH $10 SPORTS PHYSICALS 

Students in the surrounding area can receive a sports physical for $10.00 which will be donated back to the athletic program at the school the student attends. Payment to be made at time of exam. Parents must be present or forms completed prior.

Immunizations are not included in sports physicals and will need an additional appointment.

Students will be seen on a first-come, first-serve basis.

*IHSA Pre-participation forms will be available at the clinics; however, since this form serves as consent, it must be completed and signed by a parent or guardian and presented at the time of the exam. Forms may also be printed off the IHSA website. Exams will not be performed without the parent or guardian’s signature. The exam is for sports physicals only and does not apply to school physicals.

No immunizations will be given. Parental Permission is required for students under 18.
