CLINIC LPN- PEDIATRIC PRACTICE
The Licensed Practical Nurse is a clinical practitioner who participates in providing patient care. He/she is able to implement and evaluate patient care specific to the age of the patient population served from newborn to the young adult. He/she demonstrates performance consistent with the mission, philosophy and goals of organization; remains flexible to changing system; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats staff, physicians, patients and families with consideration and respect. Acts as a patient advocate at all times. To assist the primary provider in all professional and clinical procedures. Clinical procedures shall include phlebotomy, performing EKG’s, urinalysis, injections, and taking and recording patient vital signs.

Pediatric Clinic experience preferred.

MATERNAL/ NEWBORN RN-FULL TIME POSITION
The Obstetrical Registered Nurse is a clinical practitioner who assesses, coordinates, implements, and evaluates patient care specific to female patients age menarche to menopause in the ante partum period, labor, delivery, post partum, and to newborn infants. He/she ensures that quality care is provided in an efficient and safe manner, consistent with the mission, philosophy and goals of the unit and organization; remains flexible to changing systems; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats the staff, physicians, patients, and families with consideration and respect, acting as a patient advocate at all times.

Will be scheduled to work 5:30PM to 6:00AM 36 hours per week and every 3rd weekend.

Labor and Delivery Experience highly preferred. Illinois license requried.

CLINICAL PATIENT ACCESS COORDINATOR
Interviews and registers patients for hospital admittance, outpatient procedures, and emergency services either in person or on the phone for pre-registration. Obtains all applicable medical, demographic, and payment source information to enable the Hospital to appropriately treat the patient, bill the proper payers, and comply with Federal and State regulations.

Assists the nursing staff by performing clerical and clinical duties within the individuals scope of care within the nursing unit as needed; records patient ER visits in the electronic statistical tracking sheets and CPSI ER log; processes patient charts and files testing in the patient’s chart before it goes to Medical Records; verifies benefits on inpatient registrations, outpatient surgical, and other accounts as needed.

36 hours per week - 5:30pm to 6:00am Friday, Saturday, and Sunday

CNA/UNIVERSAL WORKER-MAPLE GROVE MEMORY CARE
The Certified Nurse Assistant is responsible for providing personal care to the residents. Personal care includes assistance with meals, dressing, movement, and bathing. The CNA follows established facility policies and procedures while providing care and coordinates resident care with other facility personnel and families.

Education and Qualification:
CNA certification with experience in long term care and dementia preferred
Also food handling training certificate required (can be obtained during first 6 months of employment)
High School Diploma
Experience with cooking desired

Days of work and shifts will vary - Weekend work as well.

CLINIC RECEPTIONIST
Performs clerical duties, which include intake and check out of patients. Provides patient appointments based on protocol and phone assessment of patient condition. Collects co-pay and deductible amounts from patients and tracks patient financial and productivity information. This position will perform clinic duties and provide clerical and administrative duties for the Diabetic Education Department.

High school degree required. Office clerical and administrative certification preferred.

Excellent customer service skills required.

Schedule is Monday-Friday 8am to 5pm
