Certified or Registered Polysomnographer/Sleep Tech
Must be able to perform and score sleep studies.
Two bed sleep lab that performs diagnostic and titration studies.
This position will work two nights per week, 12 hour shifts. The nights are flexible.

This individual will assist with all administration duties.
Tours & Marketing.
Billing related to private individuals, long term care insurance, veterans and Medicaid.
Physician and benefit coordination.
Assist with all applications for benefit utilization.
Assist with new employee on boarding and education.
Assist with regulatory compliance and all survey processes.
Assist with scheduling.

Qualifications: A degree in Social Services or Health Management is required.

Experience with the following is preferred:
Social Services, Medicaid, Veterans Administration, Medicare, MDS, Long term Care Insurance, HFS, IDPH, USDA, Housing and supervision of employees.

RN MED/SURG
The Registered Nurse is a clinical practitioner who assesses, coordinates, implements and evaluates patient care specific to the age of the patient population served from newborn to the elderly. He/she ensures that quality care is provided in an efficient and safe manner, consistent with the unitís standards of care. He/she demonstrated performance consistent with the mission, philosophy and goals of the unit and organization; remains flexible to changing system; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats staff, physicians, patients and families with consideration and respect. Acts as a patient advocate at all times.

Clinic Medical Assistant
This is a clinical practitioner who participates in providing patient care. Graduate of a Medical Assistant program with the appropriate Illinois licensure.

Clinic experience preferred.

Schedule is Monday-Friday.

CNA/UNIVERSAL WORKER-MAPLE GROVE MEMORY CARE
The Certified Nurse Assistant is responsible for providing personal care to the residents. Personal care includes assistance with meals, dressing, movement, and bathing. The CNA follows established facility policies and procedures while providing care and coordinates resident care with other facility personnel and families.

Education and Qualification:
CNA certification with experience in long term care and dementia preferred
Also food handling training certificate required (can be obtained during first 6 months of employment)
High School Diploma
Experience with cooking desired

Part Time Cook
Job Description:
Prepare and serve meals in a sanitary manner to preserve nutritional quality, comply with diet orders and meet the needs of the facility.
Job Responsibilities:
Prepare meal items as ordered or directed that are safe, nutritious, of appropriate temperature and according to therapeutic diet guidelines. Prepares pureed and ground meat entrees, pureed vegetables and desserts needed for residents.
Maintain clean and sanitary work area and equipment. Completes assigned cleaning in a timely manner.
Clean and sanitize dishes and utensils.
Completes temperature checks and records on daily log sheet.
Plan and prepares food appropriate to age, cultural background, religious belief and diet order of residents.

Education requirements:
High School graduate or equivalent.
Food Service Sanitation Certificate from IDPH preferred.

Works 24 hours per week.
Rotating weekends.
Position is at Hickory Grove.

Nurse Care Coordinator
The Care Coordinator is responsible for functioning in an expanded role to facilitate care through interdisciplinary collaboration and care coordination. Responsible for communicating with patients and caregivers to ensure improved care transitions, enhancing patient to provider communication to identify opportunities for care continuity, encouraging the patientís self-management skills and understanding the healthcare system to guide and empower patients to participate in their own care plan.

1. Help establish the Chronic Care Management/Patient-Centered Medical Home Programs.
2. Continually assess and provide suggestions to improve Care Plans.
3. Document all patient care interactions in Electronic Health Record.
4. Must have superb time management skills.
5. Continually assess and improve relationships with all healthcare resources.
6. Must have superb assessment skills and be able to relay those to the healthcare provider in a professional manner.
7. Be able to float from the hospital and the clinic to help with the coordination of care upon discharge.

Education & Experience:
1. Current Illinois RN or LPN license.
2. Strong background in integrated healthcare delivery.
3. Care Coordination or Case Management experience preferred.

Lead Culinary Specialist
The person we are looking for is passionate about food and cooking. They are creative and proficient in food preparation and ultimately wants to please people through food.
The primary responsibility of this position is to prepare and serve palatable, nutritionally balanced quality food to residents in a sanitary manner, comply with diet orders and meet the needs of the facility.
