Description

Openings in Colchester, LaHarpe, Hamilton and Carthage.

This is a clinical practitioner who participates in providing patient care. He/she is able to implement and evaluate patient care specific to the age of the patient population served from newborn to the elderly. He/she ensures that quality care is provided in an efficient and safe manner, consistent with the unit's standards of care. He/she demonstrated performance consistent with the mission, philosophy and goals of the unit and organization; remains flexible to changing system; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats staff, physicians, patients and families with consideration and respect. Acts as a patient advocate at all times. To assist the primary provider in all professional and clinical procedures. Clinical procedures shall include phlebotomy, performing EKG's, urinalysis, injections, and taking and recording patient vital signs.

Description

The Certified Nurse Assistant is responsible for providing personal care to the residents. Personal care includes assistance with meals, dressing, movement, and bathing. The CNA follows established facility policies and procedures while providing care and coordinates resident care with other facility personnel and families.

The Maternal Newborn Registered Nurse is a clinical practitioner who assesses, coordinates, implements and evaluates patient care specific to female patients age menarche to menopause in the ante partum period, labor, delivery, post partum and to newborn infants.

Nurse Care Coordinator

Description

The Care Coordinator is responsible for functioning in an expanded role to facilitate care through interdisciplinary collaboration and care coordination. Responsible for communicating with patients and caregivers to ensure improved care transitions, enhancing patient to provider communication to identify opportunities for care continuity, encouraging the patient’s self-management skills and understanding the healthcare system to guide and empower patients to participate in their own care plan.

  1. Help establish the Chronic Care Management/Patient-Centered Medical Home Programs.
  2. Continually assess and provide suggestions to improve Care Plans.
  3. Document all patient care interactions in Electronic Health Record.
  4. Must have superb time management skills.
  5. Continually assess and improve relationships with all healthcare resources.
  6. Must have superb assessment skills and be able to relay those to the healthcare provider in a professional manner.
  7. Be able to float from the hospital and the clinic to help with the coordination of care upon discharge.

Phlebotomist

Description

Collects blood from patients, checks quality of specimen and assumes responsibility for the safety and comfort of patients following established standards and practices.

Work every Sunday 6 - 2:30.  Monday - Thursday 10:00 to 6:30.

Release of Information Clerk

Minimum education level-high school, vocational training or related field or two years experience in similar office setting. Knowledge of filing skills, medical terminology.
Description

Provides clerical duties in the functions of department for release of information, maintenance and retention of medical records for inpatients and outpatients as well as coordinating completion of records.

  1. Responds to routine inquiries regarding patient medical record information by telephone or in writing concerning release of medical record information.
  2. Ensure all requests for release of information are authorized in accordance with Release of Information policies and procedures before information is released.
  3. Demonstrates an understanding of the medicolegal implications and responsibilities related to filing and release of patient information to protect the patient and the institution.
  4. Operates designated computer terminal, typing, copying and filing equipment as directed to complete assignments.
  5. Maintains a log of all requests received and posts payment received.
  6. HIPAA tracking of disclosures.
  7. Faxing medical information in emergencies
  8. Requests information from other health care providers.
  9. Follows policies and procedures to contribute to the efficiency of the filing/release and retrieval of information and analysis of records within the department.
