Description

To manage and direct all aspects of a foodservice operation serving an elderly population living in a supportive and assisted living community

 

Primary Job Responsibilities:

1.      Interviews tenants and family members of the community to discuss the meal service and determine special needs as applicable.

2.      Provides basic nutrition education related to specific disease conditions and allergies as identified by the registered dietitian.

3.      Completes appropriate progress notes in the medical record of interaction with tenants.

4.      Completes meal observations daily and plate waste studies on a regular basis.

5.      Reviews and develops menus for daily and special event meals using a menu program and independently in compliance with applicable regulations.

6.      Conducts inventory and prepares orders to obtain adequate supplies preparation of daily menu in each community.

7.      Conducts routine meetings and in-service education with staff to enhance knowledge and promote a positive working environment

Description

Provides leadership, supervision and administration of activities and operations of Nursing Services and Clinical Support Services to assure that the philosophy, policies, procedures and practices of these departments are consistent with the Mission, Vision and Values of Memorial Hospital, JCAHO and State and Federal regulations.  Coordinates the development of organization wide plans to provide nursing care, treatment and services.

Description

Performs clerical, administrative and operational support for the organization.   Position expectations are to provide support to diabetes education department and the medical staff.  Strong, motivational and organizational skills are necessary to fulfill the varied responsibilities as noted and expected in this position.

 

Primary Job Responsibilities:

  1. Provides clerical support to the medical staff including but not limited to: meeting preparation, minute taking, Bylaw & policy and procedure updates, and tracking of all medical staff administrative actions.
  2. Develops and maintains an efficient and effective process for medical staff credentialing/privledging including initial appointments and reappointments.
  3. Provides administrative support to the diabetes education department.  Scheduling, data entry/EPIC preparation for class and other meetings.  Note taking at meetings, supply ordering preparation of reports.
  4. Develops and maintains an efficient and effective process for running the office operations including but not limited to:  maintaining conference room meeting schedule, opening/sorting mail, scheduling meetings, maintaining appropriate records, answering telephones, and circulating necessary correspondence.

Job Summary:

The Certified Nurse Assistant is responsible for providing personal care to the patients under the supervision of the Charge Nurse.  Personal care includes assistance with meals, dressing, movement, and bathing.  The CNA follows established facility policies and procedures while providing care and coordinates patient care with other facility personnel and families.

  1. Provides or assists in bathing, grooming, toileting, and/or showering, while ensuring privacy and safety and encouraging independence.
  2. Makes beds, straightens rooms, and keeps patient care areas in an organized and clean manner, ensuring the entire nursing floor is kept orderly and demonstrating pride in the facility.
  3. Respects patient’s rights by knocking on doors before entering, addressing patients by name with the appropriate title, allowing/encouraging patients to make their own decisions when appropriate
  4. Provides safe patient transfers involving bed, wheelchair, and/or shower chair, uses the gait belt with all transfers. Protects patients from complications and injuries
  5. Takes and records temperature, pulse, and respiration as required.
  6. Sets up meal trays and assists with feeding when necessary.  Assesses for choking and responds appropriately.
  7. Ensures patients and equipment are clean after meals, including hands, and removes adult clothing protectors.

Job Summary:

Monitors compliance with Federal and State regulations; Verifies all insurance on scheduled services; Completes an estimate of the self pay portion to the patient; Obtains pre-authorization/pre-certification per payer requirements for scheduled and non-scheduled hospital and clinic services prior to services rendered and ensures authorization number is documented in the appropriate field in the system; Monitors the daily operations of Financial Counseling for scheduled services.  

 

Notifies the appropriate Director of any problems in scheduling a patient or procedure; Assists the nursing staff by performing clerical duties on scheduled services as needed; Maintains effective communications with physicians, physician’s offices, ancillary departments, other departments, and co-workers as is appropriate; Reports all Financial Counseling, pre-authorization, pre-certification, and/or registration issues to the Director of Patient Financial Services.

 

Primary Job Responsibilities:

  1. Verifies all insurance on scheduled services and provides an estimate of the self pay portion to the patient.
  2. Obtains pre-authorizations/pre-certification per payer requirements for scheduled and non-scheduled hospital and clinic services prior to services rendered and ensures authorization number is documented in the appropriate field in the system.
  3. Ensures the clinical documentation is provided to the insurance when appropriate for any and all IP and OP services.
  4. Monitors the approval of all observation, inpatient and swing bed patients for adequate approval of covered days.  Calls the insurance company when appropriate to request additional days.
  5. Inputs information for inpatient stays into the Medicaid IP Health Solution system for pre-certification approval.
  6. Completes referral documentation and forms for patients being referred from Memorial Medical Clinics to a specialist.
